Tender for the Supply of Rehabilitation Equipment to a State Medical Institution

The Charity Fund "BLAGOMAY" (hereinafter - the Fund or Fund "BLAGOMAY") announces a tender for the supply of rehabilitation equipment to medical institutions in Ukraine, according to the List below. The tender is held with the aim of purchasing medical equipment and donating it for charitable purposes.

• Kinesiotherapy trainer (60x60 frame), 105 kg, plus additional configuration - 1 trainer

The Fund "BLAGOMAY" will fully purchase the items from one supplier or partially from multiple suppliers (if the entire scope cannot be covered by one supplier) selected based on this tender. The purchase will be completed within 10 business days after the tender ends.

Tender Participant Requirements:

1. A participant can be a legal entity or an individual entrepreneur (FOP) engaged in the supply of medical equipment.
2. Participants must provide information in the following format:
   • Full name of the participant, EDPRPOU (tax ID), address, phone number and contact person’s details, email address.
3. Participants must submit electronic copies of registration documents: Extract from the EDPRPOU, VAT payer extract, single tax payer extract (if available), licenses (if applicable), authorization documents such as a protocol on the appointment of the director, power of attorney copy, and bank account opening certificate.
4. Copies of documents confirming the compliance of the goods with necessary standards (compliance declarations, conformity assessment certificates, analysis certificates) or other documents proving compliance with relevant regulations.
5. A letter of guarantee or a letter from the manufacturer or authorized representative confirming the ability to supply, service, and provide training on the equipment.
6. At least 3 recommendation letters from previous clients detailing the equipment supplied by the participant.
7. Participants cannot be suppliers offering goods manufactured in Russia or Belarus.

Product Requirements:

1. The product must be new and unused.
2. Packaging must be intact and undamaged.
3. Delivered goods must meet the characteristics specified in the supplier’s commercial offer.
4. The price of the product must correspond to the mid-range price category.

How to Apply:

1. Submit all documents described in the "Tender Participant Requirements" section via email by 24:00 on the tender end date.
2. Include commercial offers detailing the equipment characteristics, photos, delivery terms, warranties, and prices.
3. Offers must be properly formatted on official letterhead, signed, and stamped (if applicable), and sent to the official email address olena.nazarchuk@charitymay.com as per the instructions.

Evaluation Criteria and Methodology:

1. Compliance of the tender participant with the tender requirements.
2. Compliance of the supplied goods with product requirements.
3. Timely submission of all necessary documents and commercial offers for each procurement item by the tender end date.
4. Goods will be procured from the supplier offering the lowest price, provided their offer fully meets the technical requirements and financial capacity of the buyer, or the supplier offering the best price-quality ratio.

Important:
The tender will be considered valid if at least 3 supplier applications are received.
Before announcing the tender, the responsible person of the Fund conducts a market assessment and submits to the Commission at least three price proposals (including price lists, screenshots, or invoices) for the requested items, along with a comparative table of offers.
Suppliers who have provided false information about themselves or have records of bankruptcy proceedings, tax arrears, or are affiliated with the Buyer, or have a real or potential conflict of interest regarding the procurement item, are not allowed to participate in the tender. This will be communicated in writing by the authorized representative of the Buyer.
