The Charitable Foundation "Blagomay" (hereinafter referred to as the Fund or "Blagomay" Foundation) announces a tender for the supply of medical equipment to rehabilitation centers and state medical institutions in Ukraine according to the List provided below. The tender is conducted with the purpose of purchasing medical equipment and donating it to the aforementioned institutions.

The "Blagomay" Foundation will purchase the items in full from one supplier or partially from several suppliers (if the entire procurement volume cannot be fulfilled by a single supplier) chosen by the Foundation based on the results of this tender selection within 10 working days from the end of the tender.
 

Requirements for Tender Participants: 

• A participant in the tender can be a legal entity or an individual entrepreneur working in the field of medical equipment supply.
• The participant must provide information about themselves in the following format: Full name of the participant, EDRPOU (Unified State Register of Enterprises and Organizations of Ukraine) code, address, phone number, contact manager's phone number, contact manager's full name, email address.
• The participant must provide registration documents in electronic format: Extract from the EDRPOU, VAT payer's extract, single tax payer extract (if applicable), licenses (if applicable), confirmation of the signatory's authority – protocol on the appointment of the head, copy of the power of attorney for the representative, certificate of bank account opening.
• Copies of documents confirming the product's compliance with necessary standards (declaration of conformity, certificate of conformity assessment, certificate of analysis) or any other document confirming compliance with the requirements related to the object of conformity assessment issued in accordance with applicable legislation.
• A guarantee letter or a letter from the manufacturer (manufacturer's representative office or branch) or a dealer/distributor authorized by the manufacturer confirming the possibility of delivering the equipment, providing service and warranty maintenance, training on equipment operation, delivery of the equipment to the medical institution, connection, and commissioning of the equipment at the medical institution.
• Recommendation letters (at least 3) from clients with a description of the equipment supplied by this supplier and the location where it was delivered.
• The tender participant cannot be a supplier offering goods manufactured in Russia or the Republic of Belarus.
   

Product Requirements: 

• The product must be new, i.e., it has not been previously used.
• The product packaging must be intact and undamaged.
• The product supplied to the medical institution must meet the specified characteristics and have the appearance indicated in the supplier's commercial offer.
• The cost of the product must correspond to the average price range.
   

How to Submit a Proposal: 

1. Submit all documents described in the "Requirements for Tender Participants" section to the email by 24:00 on the specified tender end date.
2. Commercial proposals should include a description of the characteristics, photos, delivery conditions and terms, guarantees, and cost of the equipment.
3. Proposals must be properly formatted (on official letterhead), signed, sealed (if available), and sent to the official address specified in the announcement, olena.nazarchuk@charitymay.com, in the manner and time specified in the announcement.
    

Criteria and Methodology for Evaluating Applications:

• Compliance of the tender participant with the requirements for participants.
• Compliance of the supplied goods with the product requirements.
• Timely submission of all necessary documents and commercial proposals for each item in the procurement list by the tender end date.
• Procurement of goods will be conducted from the supplier offering the lowest price, provided that their proposal fully meets the technical requirements and financial capacity of the Customer, or from the supplier offering the best price-quality ratio (economically most advantageous proposals), provided that their proposal fully meets the technical requirements and financial capacity of the Customer.
   

Important:

The tender is considered valid if at least 3 applications have been received from suppliers.
Before announcing the tender, the responsible person of the Fund conducts a market assessment and submits to the Commission at least three price offers (including price lists, screenshots, or invoices) for the goods specified in the order and a comparative table of proposals.
Suppliers who provide false information about themselves, who have records of bankruptcy proceedings, tax debt, or who are related parties to the Customer, or who have a real or potential conflict of interest regarding the procurement object, are not allowed to participate in the tender. They will be notified in writing by the authorized person of the Customer.
