Tender for Supplying Equipment to a State Medical Institution
The Charitable Fund "BLAGOMAY" (hereinafter referred to as the Fund or "BLAGOMAY" Fund) announces a tender for the supply of rehabilitation equipment to a medical institution in Ukraine in accordance with the list below. The tender is conducted to procure medical equipment and donate it for charitable purposes.
- Tender start date: January 2, 2025
- Tender end date: January 21, 2025
Procurement Item:
- Automatic rehabilitation device for legs and arms – 1 unit
The "BLAGOMAY" Fund will procure the goods either fully from one supplier or partially from several suppliers (if a single supplier cannot fulfill the entire order). The suppliers will be selected based on the tender results, and the procurement will be completed within 20 business days from the tender closing date.
Requirements for Tender Participants:

The participant can be a legal entity or an individual entrepreneur (IE) operating in the field of medical equipment supply.

The participant must provide the following information:
- Full name of the participant
- Unified State Register of Enterprises and Organizations of Ukraine (EDRPOU) code
- Address of the participant's location
- Contact phone number and responsible manager's phone number
- Full name of the responsible manager
- Email address

The participant must submit registration documents electronically:
- Extract from EDRPOU
- VAT payer extract (if applicable)
- License (if applicable)
- Proof of signatory authority (e.g., protocol on appointing the head, a copy of a power of attorney, or a bank account opening certificate).

Copies of documents confirming compliance of the goods with required standards (e.g., declarations of conformity, conformity assessment certificates, analysis certificates) or other documents meeting legislative requirements.

A guarantee letter or a letter from the manufacturer (its representative, branch, or authorized dealer/distributor) confirming:
- Possibility of supply
- Service and warranty maintenance of the equipment
- Training on the equipment usage
- Delivery of the equipment to the medical facility
- Installation and commissioning of the equipment at the facility

At least three recommendation letters from clients, describing which equipment was delivered and to which institutions.

Participants cannot offer products manufactured in Russia or Belarus.
Requirements for the Goods:
- The goods must be new (i.e., not previously used).
- The goods must have intact and undamaged packaging.
- The goods delivered must meet the specified characteristics and visual appearance outlined in the supplier's commercial proposal.
- The goods must fall within a mid-range price category.
How to Submit an Application:
- Submit all documents listed in the "Requirements for Tender Participants" section via email before the tender closing date (by 24:00).
- Attach commercial proposals detailing specifications, photos, delivery conditions and terms, warranty, and equipment costs.
- Ensure proposals are properly formatted on official letterhead, signed, and sealed (if applicable). Send them to the official email address: olena.nazarchuk@charitymay.com as specified.
Evaluation Criteria and Methodology:
- Compliance with the tender participant requirements.
- Compliance of the supplied goods with the specified requirements.
- Submission of all necessary documents and proposals on time, as per the procurement item requirements.
- Selection is based on:
- The lowest price that meets technical and financial requirements
- The best price-quality ratio (economically most advantageous offers)
Important Information:
- The tender is considered valid if at least three bids are received.
- Before announcing the tender, the responsible Fund representative will assess the market and present the Commission with at least three price proposals (e.g., price lists, screenshots, or invoices) for the listed items and a comparative table.
- Suppliers providing false information or with bankruptcy violations, tax debts, connections to the procuring entity, or conflicts of interest will be disqualified and informed in writing.
