Tender for the Supply of Equipment to a State Medical Institution
The charitable foundation “BLAGOMAY” (hereinafter referred to as the Foundation or the “BLAGOMAY” Foundation) announces a tender for the supply of rehabilitation equipment to medical institutions in Ukraine, as listed below. This tender aims to procure medical equipment for charitable purposes and its subsequent donation.

Procurement Subject
- Basic neonatal incubator – 1 unit
The BLAGOMAY Foundation will purchase the entire volume of goods either from a single supplier or from several suppliers (if the total volume cannot be covered by one supplier). Suppliers will be selected based on the results of this tender process, with procurement completed within 20 working days after the tender's conclusion.
Requirements for Tender Participants

Eligibility:
The participant must be a legal entity or an individual entrepreneur (IE) operating in the field of medical equipment supply. -
Required Information:
Participants must provide the following:- Full name of the participant, EDRPOU code, registered address, contact phone number, name and phone number of the responsible manager, and email address.

Documentation:
Participants must submit the following in electronic form:- Registration documents (extract from EDRPOU, VAT payer certificate, single tax payer certificate, if applicable).
- Licenses (if applicable).
- Proof of authority for the signatory (protocol of director's appointment, copy of the proxy, or bank account details).

Product Compliance Documents:
Copies of documents confirming the product's compliance with required standards (declaration of conformity, conformity assessment certificate, or analysis certificate) or any other document confirming compliance with applicable legal requirements. -
Guarantee Letters:
A letter from the manufacturer (or its representative, dealer, or distributor) confirming the ability to supply the product, provide service and warranty maintenance, conduct training on equipment usage, deliver equipment to the medical institution, and perform installation and commissioning at the medical facility. -
Recommendation Letters:
At least three letters from clients describing the equipment supplied and its delivery location. -
Restrictions:
Suppliers offering products manufactured in Russia or Belarus are not allowed to participate.
Product Requirements

Condition:
- The product must be new and unused.

Packaging:
- The packaging must be intact and undamaged.

Compliance:
- The product supplied must meet the specified characteristics and appearance as outlined in the supplier’s commercial offer.

Pricing:
- The cost should correspond to the mid-range pricing category.
How to Submit an Application
- Submit all required documents (as described in the "Requirements for Participants" section) via email by 24:00 on the tender end date.
- Include commercial offers specifying product characteristics, photos, delivery terms, guarantees, and pricing.
- Ensure that offers are properly formatted (on official letterhead, signed, and stamped, if applicable) and sent to the official email olena.nazarchuk@charitymay.com as specified in the announcement.
Evaluation Criteria and Methodology
- Compliance of the participant with the tender requirements.
- Compliance of the product with the specified requirements.
- Timely submission of all required documents and commercial proposals for each item in the procurement subject.
- Preference will be given to the supplier offering the lowest price, provided that their offer meets all technical requirements and financial capabilities of the purchaser, or to the supplier providing the best price-to-quality ratio (economically most advantageous proposals).
Important Notes
The tender will be considered valid if at least three applications are received.
Before announcing the tender, the responsible person from the Foundation will conduct a market analysis and submit to the Commission at least three price proposals (including price lists, screenshots, or invoices) for the specified goods, along with a comparative table of proposals.
Suppliers providing false information, having records of bankruptcy violations, tax debt, or a conflict of interest with the purchaser, or those associated with the purchaser, will not be admitted to the tender. Such participants will be informed in writing by the authorized representative of the purchaser.
