Tender for the Supply of Equipment to a State Medical Institution
The charitable foundation “BLAGOMAY” (hereinafter referred to as the Foundation or “BLAGOMAY” Foundation) announces a tender for the supply of rehabilitation equipment to medical institutions in Ukraine according to the list provided below.
The tender is conducted for the purpose of purchasing medical equipment and donating it as charitable aid.

Procurement subject:
Wound cleansing system; endoscopic video processor and disposable ureterorenoscope sets (flexible video ureterorenoscope). X-RAY monoblock.
The “BLAGOMAY” Fund will procure the full volume of goods either from a single supplier or partially from several suppliers (if one supplier cannot fulfill the entire scope), selected based on the results of this tender, within 20 working days from the tender’s closing date.
Requirements for Tender Participants

The participant may be a legal entity or an individual entrepreneur engaged in the supply of medical equipment.

The participant must provide information in the following format:
Full name of the participant, EDRPOU code, registered address, phone number and phone number of the responsible manager, full name of the responsible manager, and email address. -
The participant must submit registration documents in electronic form: Extract from the Unified State Register (EDRPOU), VAT payer certificate, single tax certificate (if applicable), licenses (if applicable), confirmation of signatory authority – minutes of appointment of the director, copy of power of attorney for the representative, and a bank account certificate.

Copies of documents confirming the conformity of goods to required standards (declarations of conformity, conformity assessment certificates, certificates of analysis), or any other document confirming compliance with applicable legal requirements.

A warranty letter or a letter from the manufacturer (its branch or representative office) or an authorized representative/dealer/distributor confirming the ability to supply, service, and provide warranty maintenance for the equipment, conduct training on its use, deliver it to the medical institution, connect, and commission it on-site.

At least three recommendation letters from previous clients, describing what type of equipment was supplied and to which institutions.

A participant cannot offer goods manufactured in the Russian Federation or Republic of Belarus.
Requirements for the Goods

The goods must be new, i.e., not previously used.

Packaging must be intact and undamaged.

The delivered goods must meet the declared specifications and appearance described in the supplier’s commercial offer.

The cost must correspond to the mid-range market price.
How to Apply

Submit all documents listed in the section Requirements for Tender Participants by email before 24:00 on the tender’s closing date.

Attach commercial proposals describing specifications, photos, delivery conditions and timelines, warranty terms, and equipment cost.

All proposals must be properly formatted (on official letterhead), signed, sealed (if applicable), and sent to info@charitymay.com in the manner and time specified in the announcement.
Evaluation Criteria and Methodology

Compliance of the participant with tender participation requirements.

Compliance of the goods with technical specifications.

Submission of all required documents and commercial proposals before the deadline.

Goods will be procured from the supplier offering the lowest price, provided the offer fully meets technical requirements and the Foundation’s financial capacity — or from the supplier offering the best value for money (economically most advantageous offer), under the same compliance conditions.
Important!
The tender is considered valid if at least three applications from suppliers are received.
Before announcing the tender, the responsible Foundation representative conducts a market assessment and submits to the Commission at least three price quotations (including price lists, screenshots, or invoices) for the goods listed in the order, along with a comparative table of proposals.
Suppliers providing false information, those under bankruptcy proceedings, having tax debt, or connected partieswith the Foundation or with actual or potential conflicts of interest related to the procurement subject will be disqualified and notified in writing by the Foundation’s authorized representative.
