The Charity Fund "BLAGOMAY" (hereinafter referred to as the Fund or the "BLAGOMAY" Fund) announces a tender for the supply of rehabilitation equipment to medical institutions in Ukraine according to the list provided below. The tender is conducted for the purpose of purchasing medical equipment and donating it.

Tender start date: October 3, 2024
Tender end date: October 22, 2024

Subject of procurement:

• Mechanical operating table, 2 units
• LED operating lamp, 2 unit

The "BLAGOMAY" Fund will purchase the goods in full from one supplier or in parts from several suppliers (if the entire volume cannot be fulfilled by a single supplier). The Fund will select suppliers based on the results of this tender process within 10 working days from the end of the tender.

Requirements for tender participants:

1. The participant may be a legal entity or an individual entrepreneur (FOP) engaged in the supply of medical equipment.
2. The participant must provide information in the following format:
   • Full name of the participant, Unified State Register of Enterprises and Organizations of Ukraine (EDRPOU) code, address, phone number and phone number of the responsible manager, full name of the responsible manager, email address.
3. The participant must submit registration documents in electronic form:
   • Extract from the EDRPOU, VAT payer certificate, Unified Taxpayer certificate (if available), licenses (if available), confirmation of the signatory's authority – minutes of appointment of the director, a copy of the power of attorney, a certificate of bank account opening.
4. Copies of documents confirming the compliance of the goods with the necessary standards (declarations of conformity, conformity assessment certificate, certificate of analysis) or any other document confirming compliance with the requirements related to the object of conformity assessment, issued in accordance with current legislation.
5. A letter of guarantee or a letter from the manufacturer (or its representative office, branch, representative, dealer, distributor authorized by the manufacturer), confirming the possibility of supplying the goods, providing service and warranty maintenance, training on how to operate the equipment, delivery of the equipment to the medical facility, and connection and launch of the equipment in the medical facility.
6. Recommendation letters (minimum of 3) from clients describing the equipment supplied by the provider and where it was delivered.
7. A supplier offering goods manufactured in the territory of Russia or the Republic of Belarus cannot participate in the tender.

Requirements for the goods:

1. The goods must be new, meaning they have not been used before.
2. The packaging of the goods must be intact and undamaged.
3. The goods delivered to the medical institution must meet the specified characteristics and have the appearance indicated in the supplier's commercial offer.
4. The price of the goods should correspond to the mid-range price category.

How to submit an application:

1. Submit all the documents described in the "Requirements for tender participants" section by email by 24:00 on the specified tender end date.
2. The documents must include commercial proposals with descriptions of characteristics, photos, delivery terms and conditions, warranties, and the cost of the equipment.
3. Proposals must be properly formatted (on official letterhead), signed, stamped (if applicable), and sent to the official email address specified in the announcement (olena.nazarchuk@charitymay.com) in the manner and by the deadline specified.

Criteria and methodology for evaluating applications:

1. The participant’s compliance with the requirements for tender participants.
2. The goods’ compliance with the requirements for the goods.
3. Submission of all required documents and commercial proposals by the deadline for each item in the procurement list.
4. Goods will be purchased from the supplier offering the lowest price, provided their offer fully meets the technical requirements and financial capacity of the purchaser, or from the supplier offering the best price-to-quality ratio (economically advantageous offers), provided their offer fully meets the technical requirements and financial capacity of the purchaser.

Important:
The tender is considered valid if at least 3 applications are received from suppliers.
Before announcing the tender, the responsible person from the Fund will conduct a market evaluation and submit at least three price offers (including price lists, screenshots, or invoices) for the specified goods to the Commission, along with a comparative table of the offers.

Suppliers who provide false information about themselves or have records of bankruptcy proceedings, tax arrears, or are affiliated with the purchaser or have a real or potential conflict of interest regarding the subject of procurement will not be allowed to participate in the tender, and they will be informed in writing by the authorized person of the purchaser.
