The Charitable Foundation “BLAGOMAY” (hereinafter – the Foundation or the “BLAGOMAY” Foundation) announces a tender for the supply of rehabilitation equipment to medical institutions of Ukraine in accordance with the List provided below. The tender is conducted for the purpose of purchasing medical equipment and transferring it as charity.

Procurement item:
Digital X-ray diagnostic system with a flat-panel detector suitable for carrying out general and targeted radiographic examinations.

The BLAGOMAY Foundation will purchase the goods in full from one supplier or partially from several suppliers (if it is not possible for a single supplier to cover the entire procurement volume). The suppliers will be selected based on the results of this tender, and the procurement will be carried out within 10 working days from the tender completion date.

Tender participant requirements
A participant may be a legal entity or an individual entrepreneur engaged in the supply of medical equipment.

The participant must provide information about themselves in the following format: full name of the participant, EDRPOU code, registered address, phone number and phone number of the responsible manager, full name of the responsible manager, email address.

The participant must provide electronic copies of registration documents: Extract from the Unified State Register (EDRPOU), VAT payer extract, single taxpayer extract (if applicable), licences (if applicable), confirmation of the signatory’s authority – Minutes of appointment of the director, power of attorney for the representative, bank account opening certificate.

Copies of documents confirming that the goods meet the required standards (declaration of conformity, conformity assessment certificate, certificate of analysis) or any other document confirming compliance with the requirements applicable to the conformity assessment object, issued in accordance with current legislation.

A guarantee letter or a letter from the manufacturer (its representative office or subsidiary) or an authorized representative, dealer, distributor confirming the ability to supply the goods, provide service and warranty maintenance, training for equipment operation, delivery to the medical facility premises, connection and commissioning at the medical facility.

Recommendation letters (minimum 3) from clients describing which equipment was delivered previously and to which institutions.

A tender participant cannot be a supplier offering goods manufactured in the territory of Russia or the Republic of Belarus.

Product requirements
The goods must be new, i.e., not previously used.
The packaging must be intact and undamaged.
The goods delivered to the medical institution must correspond to the characteristics and visual appearance specified in the supplier’s commercial offer.
The cost of the goods must correspond to the mid-price range.

How to apply
Submit all documents described in the “Tender participant requirements” section via email by 24:00 on the specified tender end date.

Commercial proposals must be attached, including a description of characteristics, photos, delivery terms and deadlines, warranty, and equipment cost.

Proposals must be properly formatted (on official letterhead), signed, stamped (if applicable), and sent to the official email address olena.nazarchuk@charitymay.com in the manner and timeframe specified in the announcement.

Criteria and methodology for evaluating applications
Compliance of the participant with tender requirements.
Compliance of the offered goods with the product requirements.
Submission of all required documents and commercial proposals on time and for each item listed in the procurement subject.

Procurement is carried out from the supplier offering the lowest price, provided that the proposal fully meets technical requirements and the Customer’s financial capacity; or from the supplier offering the best price-quality ratio (the most economically advantageous offer), provided that the proposal fully meets technical requirements and the Customer’s financial capacity.

Important
The tender is considered valid if at least 3 applications from suppliers are received.

Prior to announcing the tender, the responsible person of the Foundation conducts a market assessment and submits to the Commission at least three price proposals (including price lists, screenshots, or invoices) for the goods specified in the order, as well as a comparative table of offers.

Suppliers who submit false information or have records of bankruptcy violations, tax debt, are affiliated with the Customer, or have an actual or potential conflict of interest regarding the procurement subject, are not allowed to participate in the tender. Such suppliers will be notified in writing by the Customer’s authorized representative.
