Tender for the supply of medical equipment to state medical institutions and rehabilitation centers of Ukraine
The "BLAGOMAY" Charitable Foundation (hereinafter - the Foundation or the "BLAGOMAY" Foundation) announces a tender for the supply of medical equipment to rehabilitation centers and state medical institutions of Ukraine according to the List below. The tender is held for the purpose of purchasing medical equipment and donating it to charity.

The "BLAGOMAY" Fund will purchase the goods in full from one supplier or in parts from several suppliers (if the entire volume of the procurement item cannot be covered by one supplier), whom the Fund will choose based on the results of this tender selection, within 10 working days from the end of the tender.

Requirements for tender participants
1. A tenderer can be a legal entity or an individual entrepreneur (entrepreneur) working in the field of medical equipment supply.
2. The participant must provide information about himself in the following format:
Participant's full name, EDRPOU, location address, phone number and phone number of the responsible manager, full name of the responsible manager, e-mail address.
3. The participant must provide registration documents in electronic form: Extract/Extract from the EDRPOU, Extract of the payer of VAT, Single tax (if available), License (if available), confirmation of the authority of the signatory - Protocol on the appointment of a manager, a copy of a power of attorney for a representative, a certificate of opening a bank account.
4. Copies of documents confirming the conformity of the product to the necessary standards (declarations of conformity, certificate of conformity assessment, certificate of analysis) or any other document confirming the fulfillment of requirements related to the object of conformity assessment and issued in accordance with the requirements of current legislation.
5. Warranty letter or letter of the manufacturer (representative office, branch of the manufacturer) or representative, dealer, distributor authorized by the manufacturer, which confirms the possibility of product delivery, service and warranty maintenance of the equipment, training on working with such equipment, delivery of equipment to the premises of a medical institution, connecting and starting equipment in the premises of a medical institution.
6. Reference letters (minimum 3 letters) from customers describing what equipment has been delivered by this supplier before and where.
7. A supplier offering goods manufactured on the territory of Russia or the Republic of Belarus cannot be a tender participant.

Product requirements:
1. The product must be new, i.e. it has not been used before.
2. The product packaging must be complete and not deformed.
3. The goods delivered to the medical institution must meet the requirements of the characteristics and have a visual appearance, which are indicated in the commercial offer of the supplier.
4. The price of the product must correspond to the average price class.
How to apply
1. Submit all the documents described in the section Requirements for tender participants by e-mail by 24:00 of the specified tender end date
2. Commercial offers with a description of the characteristics, photos, conditions and terms of delivery, warranty and cost of the equipment must be added to the documents.
3. Proposals must be duly completed (on official letterhead), signed, sealed (if available) and sent to the official address tender@charitymay.com specified in the announcement, in the manner and time specified in the announcement.

Criteria, application evaluation methodology
1. Compliance of the tender participant with the requirements for participants
2. Conformity of the supplied goods to the requirements for the goods
3. All necessary documents and commercial proposals for each item in the Subject of procurement have been submitted on time, before the end date of the tender.
4. The purchase of goods is made from the supplier offering the lowest price, provided that his offer fully meets the technical requirements and financial capacity of the Customer, or from the supplier that provides the best price-quality ratio (the most economically advantageous offers), provided that his the offer fully meets the technical requirements and financial capacity of the Customer.
